Fees for services are $5 per doctor visit, $10 (generic), $20 for formulary name brand, and $30 for non-formulary name brand prescriptions, and lab and x-ray services for $5. Unlimited doctor visits, prescription medication, lab tests, and x-rays, as defined for primary care for residents of Suburban Cook County with a family income of less than 200% of the federal poverty guidelines. Must be ineligable for public aid, Medicare, Kid Care Share, have no health insurance (or a $500 or more deductable). Low co-payments for services.
